Ok, so your hda5 is hosting the grub install, and contains a / filesystem
for lfs 5.1 . (and the kernel is in /boot/kernel-2.6.4) What partion are you
trying to mount as root? As in:

kernel (hd0,4)/boot/kernel-2.6.4 root=hd??
