There are substantial differences between the inittab you show here and
the once detailed in the current LFS (4.0) book. You say you copied lfs
3.1 script, but you do not say what version ov LFS you are making/running
now. I suggest you make your inittab look exactly like the current book,
as a starting point, and then modify carefully to meet your needs.

Obvious (and major?) differences: you have no initdefault entry, your
agetty setups are different (did you enable virtual console support in the
kerenel, did you make /dev/tty* nodes?), your "su:" entry is different and
so on.
