I have the same kind of modem, and had to use the rpm here to make it work...
You'll probably have to use rpm2cpio or rpm2tgz to extract the files
edit the serial init file, and change the line
then create this symlink to run it on every startup
ln -s ../init.d/serial S75serial
mknod /dev/ttyS4 c 5 67
Will make the device file you need
