Another possibility if you're using /dev/pts, missing or incorrect
permissions on /dev/ptmx

crw-rw-rw-   1 root     root       5,   2 Nov 27 06:42 /dev/ptmx

mknod /dev/ptmx c 5 2
chmod 666 /dev/ptmx
