Ok I'll tell you about two more I found 8):

No such file or directory looking for "asm".
The start of chapter06/kernel.xml has:

<package name="linux-headers" version="&linux-version;">
	<stage name="Installing kernel headers.">
		<copy>
			<source>/tools/include/asm/*</source>
			<destination>/usr/include/asm</destination>
		</copy>

I used:

<package name="linux-headers" version="&linux-version;">
	<stage name="Installing kernel headers.">
		<copy>
			<option>recursive</option>
			<source>/tools/include/asm</source>
			<destination>/usr/include</destination>
		</copy>

No such file or directory looking for "make_devices-1.2"
chapter06/makedev.xml has:

		<permissions base="/dev" mode="754">
			<name>&make_devices-file;</name>
		</permissions>

I used:

		<permissions base="/dev" mode="754">
			<name>make_devices;</name>
		</permissions>

It seems to be ok with what I used, in that it got much further (it stopped
during make test of glibc, then the hard drive died a few packages on -
typical).
